摘 要

随着改革开放的不断深入和社会主义市场经济的快速发展，我国医药行业也在稳步发展，其在满足公众日益增长的医疗需求的同时，“看病难，看病贵”问题在不断凸显。其中，药价虚高、不合理用药现象不仅导致药品费用的大幅上涨，也严重危害了患者的身体健康。因此，如何降低药价、如何规范用药己成为政府面临的重要问题，这也在一定程度上催生了“建立国家基本药物制度”的设想。通过对比别国的基本药物质量监管体系、定价体系、报销体系、采购制度和诊疗标准等诸多方面，了解到我国的基本药物制度的缺陷，以及应从哪些具体方面完善改进。

Comparative study on the system of essential drugs at home and abroad

Abstract

With the deepening of reform and opening up and the rapid development of the socialist market economy, China's pharmaceutical industry is also developing steadily. While meeting the growing medical needs of the public, the problem of "difficult to see a doctor and expensive to see a doctor" is constantly highlighting. Among them, the phenomenon of unreasonably high drug prices and unreasonable drug use not only led to a substantial increase in drug costs, but also seriously endangered the health of patients. Therefore, how to reduce the price of medicines and how to regulate the use of medicines has become an important issue facing the government, which has also to some extent spawned the idea of "establishing a national essential medicine system." By comparing the essential drug quality supervision system, pricing system, reimbursement system, procurement system and diagnosis and treatment standards of other countries, we learned the shortcomings of China's essential drug system and what specific aspects should be improved.

1.1.1基本药物的概念

二十一世纪初，世界卫生组织为了更加精准地阐述出基本药物制度的概念，把基本药品改为了基本药物 ，即基本药物为了满足公众最需要的药品。选择的依据是人们的健康，这是有效，安全且相对划算的。在运作良好的医疗卫生体系中，基本药物应具有足够的数量，适当的剂型，质量保证，足够的信息和可接受的价格。^[1]我国将其定性为适应医疗和卫生保健的需要，具有适当的剂型和合理的价格，可以确保向公众提供的药物。世卫组织的定义突出了基本药物筛选阶段中的循证原则，使挑选过程更加公开、公正，具有合理性。但中国的定义偏向简单，没有具体体现出基本药物的内容和性质。

1.1.2基本药物的遴选原则

基本药物的筛选和目录更新任务。该委员会根据以下标准选择基本药物。

(1)首先，应当考虑的是各国的常发病例，尤其是疾病谱的状况。

(2)应当挑选在各个医疗卫生机构中经常使用或在临床治疗中安全有效的药品。
